Registration requirements for families who received and accepted a placement offer to ASI via RSCO:

1.      Student's long form birth certificate or Passport
   
2.      Parent/Guardian State Photo ID, Driver’s License or Passport
3.      Current student transcript (H. S. applicants entering grades 10 & 11)*
4.      Any two proofs of residency dated within 90 days:

·  A current apartment or home lease agreement (first page and signature page), a mortgage document, property tax record, rent receipt (must show address along with landlord name and number), home owners insurance

· A current utility bill (electric, gas, water) cable, home or cell phone bill, insurance correspondence. Bills must show the service address. If using an Eversource bill please include the second page that shows the service address in the blue box.

· A current proof of government benefits (Disability, Medicare, Supplemental Assistance Nutrition Program (SNAP) or other government correspondence showing address

·   Automobile registration or automobile insurance

5. A copy of student’s most recent physical which should be faxed to 860-893-0073 or e-mailed to [email protected]

All proofs of residency must be of the complete/entire bill; not folded, torn or in an envelope. The bills cannot be dated more than 90 days from the time of registration.

If a family resides at a home which the parent/guardian name does not appear on the mortgage and the family does not receive bills/documents in their name, it is important that they contact the school registrar to receive further instructions.

*For all students who are enrolling into the 10th & 11th grade, a transcript is required since the seat offer is contingent upon the student meeting the required credit for the incoming grade.
